1) Provide me with the rider information below 2) Fill out the waiver form (click for pdf file) and fax it to (425) 483-0327. |

Once that is complete, by filling out and faxing the waiver form you will officially be a rider for the event. Rider Information 1) First Name, Last Name of Rider 2) First Name, Last Name of Passenger 3) Street Address 4) City, State Zip Code 5) Phone Number and E-mail address 6) Are you okay with me putting your name, City, and e-mail on the roster posted on the web page? Yes or no. 7) Bike Make and Model 8) Touring Experience (1 year, 2 years, 3 years, or none, some or lifer) 9) Looking to share room expenses? 10) Number of miles you will ride on this trip.
